Case management is a service that assists an individual in gaining access to needed medical, social, educational, and/or other services. It may be used to locate, coordinate, and monitor necessary and appropriate services, and to support referrals to appropriate providers.

Case management may also help coordinate non-medical services — such as education, employment-related resources, or community supports — when those services help a person function at their highest level.

Case management is a coordination and navigation service. It is not medical treatment, therapy, or an emergency service, and it does not guarantee eligibility, enrollment, or outcomes for any program.

The Role of Family & Caregivers

Families and caregivers are often central to the case management process. With appropriate permission, we work to keep families informed, gather helpful context about the situation, and include caregivers in coordination conversations.

The Role of Healthcare & Community Organizations

Providers, schools, and community organizations may refer individuals for case management support, and may be contacted as part of coordinating appropriate services — always with appropriate communication and consent.
